Long-Range Goals

 


BENCHMARK OBJECTIVES


1. Collaborative development and sustainment efforts will provide community residents with 100% access to available and affordable resources through sound fiscal management, diverse board and committee representation, and outcome based policies and practices.

 

a) Increase from 4 in 2003 to 6 in FY 2006, major collaborative partnerships that are in place and can be demonstrated, which allow services to be provided that would not exist without the collaboration.

b) Increase implementation of long-term funding plan from one strategy in 2003 to three diverse strategies by FY 2006.


c) Increase implementation of the collaborative's evaluation plan to include process data and results and outcomes from one strategy area in 2003 to all three areas by FY 2006.

d) Increase the total number of collaborative volunteerfrom 85 in 2003 to 100 by FY 100.

 

2. All children will remain free of abuse and neglect.

a) Reduce rate of confirmed incidents of abuse and neglect from the current rate per 1,000 children under 18 from 50.3 in 2001 to 35.0 by FY 2006. (2002 GA County Guide)

b) Reduce the percent of live births to unwed mothers from 57.9 in 1999 to 45.0 by FY 2006

*Working with DFCS to obtain figures for these CPPC cross-site evaluation benchmarks.

 

3. All children will graduate from high school.


a) Increase % of students entering 9th grade and graduating 4 years after high school enrollment from 71.8% in 2002 to 79.8% by 2006. (2002 GA County Guide)

 

4. All children and adults will have 100% access to healthcare with 0% disparities in available treatment.

a)
Reduce the total deaths from heart disease from 39.3 in 1999 to 25.0 by FY 2006; Cancer from 13.1 in 1999 to 10.0 by FY 2006; and Respiratory Disease from 6.0 in 1999 to 3.0 by FY 2006. (2002 GA County Guide)
